As Dr. Blocker notes: “Fluoroscopy is great for looking at bony structures like joints, while ultrasound gives us a real-time picture of bone, ligaments, muscles, and nerves.”

What Are the Benefits of Image Guidance Over Blind Injection Techniques?

In the past, musculoskeletal injections were sometimes performed “blind,” using only anatomical landmarks as a guide. While this approach may be practical, it carries a greater risk of missing the target or causing complications.

With imaging, injections are far more precise:

How Long Does Relief Last, and Are Multiple Treatments Needed?

Relief from a lumbar spine injection or other spinal injection varies depending on the condition and individual patient. On average, patients can expect about three months of improvement following a steroid injection. Some patients experience six, nine, or even 12 months of relief.

“Success is roughly three months after a steroid injection,” Dr. Blocker explains. “But many patients can get longer-lasting relief.” Repeat injections may be recommended, but the overall goal is to restore function and reduce pain with the fewest procedures necessary.

How Do You Determine Which Type of Image Guidance or Injection Is Right for the Patient?

The choice between fluoroscopy and ultrasound depends on the condition being treated:

  • Fluoroscopy: Best for targeting larger joints or bony structures such as discs or arthritic joints.
  • Ultrasound: Best for soft tissue, ligament, or nerve-related pain where real-time visualization is critical.

“It depends on what we are treating,” says Dr. Blocker. “If we are targeting larger joints, we will use fluoroscopy because we can target the disease process right at the bone. If we are treating ligaments or soft tissue, we use ultrasound because we can see those tissues moving in real time.”
